About Transfer of Care
Welcome to the Transfers of Care Network
Improving transfers of care is a priority for the health and care systems across the East and a commitment of the NHS Long Terms Plan. The network aspires to the ambitions of the Five Year Forward View and Quality Matters which includes:
- Sharing practice and improving how hospitals work in partnership with social care services so that people are supported to leave hospital with quality personalised care;
- Discussing and implementing elements of the Local Government Association’s High Impact Change Model for reducing Delayed Transfers of Care;
- Working across health and social care to reduce delayed transfers of care using Home First and Discharge to Assess principles.
